A fracture of the patella (kneecap) is most common with a direct blow, if falling directly on the patella, or rarely with incredibly strong quadriceps contractions when the lower leg is fixed in position.
The most common symptom is pain directly over the patella. In some cases, one can see a divot or dimple in the middle of the patella if the fractured parts are separated from each other. There will be pain when straightening the knee.
An Xray of the knee is most commonly used to diagnose a patellar fracture. If the fractured edges are not separated by more than 3-4 mm, a knee immobilizer can be worn for 2-3 weeks, followed by progressive range of motion exercises. For more displaced fractures (more than 3-4 mm), surgical repair is recommended.
