I think that the remainderman could still be responsible for the hospital debt, if the person has any life insurance I think it could come from there. But in North Carolina, where I am from,a home could not be foreclosed due to medical expenses, they show on your credit but usually can not effect it so we speak. It is a necessity(health care) almost everyone has medical debt, I wouldn't worry too much. I have worked in the medical field and now I am a broker and I wouldn't be too concerned. Sometimes before closing on a new home they will have you prove that you are paying on it or pay it off, but in the occurance of a death that is different.
The debts of the deceased are the responsibility of the estate. The hospital can certainly put a lien against the estate for their money. Anyone that was also a co-signer on any of the agreements might also be responsible. Consult a probate attorney in your jurisdiction for help.
