Nothing. Many parts of the world still don't use deodorant. The United States is one of the few that does. A thousand years ago people always smelled like wood smoke and tallow since that is what lit and heated their homes.
They used to carry round perfumed objects such as 'pomanders' (see related link).
Also people got used to bodily odors as they still do today in developing countries.
