Seizure First AidThe first aid for a seizure depends on the type of seizure occurring. Generalized seizures will cause the person to fall, which may result in injury. A tonic-clonic seizure results in violent movements that cannot and should not be suppressed. The person should never be restrained, nor should there be any attempt to put something in the mouth. Potentially sharp or dangerous objects should also be moved from the vicinity, so that the individual is not hurt. After the seizure if the person is not fully conscious and alert, they should be placed in the recovery position, unless you suspect spinal cord injury1.

Recovery PositionThe recovery position is a first aid technique recommended for assisting people who are unconscious, or nearly so, but are still breathing. It is frequently taught as part of classes in CPR (cardiopulmonary resuscitation) or first aid. When an unconscious person is lying face upwards, there are two main risks factors which can lead to suffocation:
  • The tongue can fall to the back of the throat, due to loss of muscular control. The back of the tongue then obstructs the airway.
Fluids, possibly blood but particularly vomit, can collect in the back of the throat, causing the person to drown. When a person is lying face up, the esophagus tilts down slightly from the stomach towards the throat. This, combined with loss of muscular control, can lead to the stomach contents flowing into the throat, called passive regurgitation. As well as obstructing the airway, fluid which collects in the back of the throat can also then flow down into the lungs; stomach acid can attack the inner lining of the lungs and cause a condition known as aspiration pneumonia It is not necessary to call an ambulance if the person is known to have epilepsy, if the seizure is shorter than five minutes and is typical for them, if it is not immediately followed by another seizure, and if the person is uninjured. Otherwise, or if in any doubt, medical assistance should be sought. The correct position is called the "lateral recovery position. Start with the victim laying on the back and with the legs straight out. Kneel on one side of the victim, facing the victim. Move the arm closest to you so it is perpendicular to the body, with the elbow flexed (perpendicular). Move the farthest arm across the body so that the hand is resting across the torso. Bend the leg farthest from you so the knee is elevated. Reach inside the knee to pull the thigh toward you. Use the other arm to pull the shoulder that is farthest from you. Roll the body toward you. When a person begins to have a seizure:

  • Do not panic.
  • Call 911 or your country's emergency service line -- early on.
  • Note the time the seizure started - you may be asked later about how long the seizure continued.
  • Move furniture or objects away from the person.
  • If you can't move furniture, put pillows between the person and any sharp edges of objects or furniture.
  • Remove small children from the area.
  • Do NOT ever try to hold the person down or hold the person still.
  • Do NOT yell at the person or try to "shake" the person out of a seizure.
  • Talk quietly, in a reassuring voice.
  • Tell the person before you touch them, especially after the seizure has ended.
  • Try to roll the person onto his / her side. Put pillows or couch cushions behind the person's back to hold the person to their side.
  • If the person has vomited, make sure the person is on his/her side. Open the person's mouth (do NOT try this during the seizure!) Use a cloth to wipe away the vomit.
  • Stay with the person!
  • If the person regains consciousness, encourage them to stay on the floor and on his/her side until paramedics arrive.
  • Do not try to give water or any other drinks.
  • Be reassuring and calm -- the person will often be confused right after a seizure. Just say, "You've had a seizure; stay laying down here."
  • Report to the paramedics the time the seizure started, the time it ended, if the person has a history of seizures, if you know that they take anti-seizure medication (anticonvulsant), and the name of a family member/phone. Answer as many questions as you can.
