The word is also spelled cay and comes from the Spanish cayo. It means a low offshore island, often made of coral.

My Muret-Sanders Encyclopaedic (and often Etymologic!) German-English and English-German Dictionary of 1908 says in its English-German Part:

key 1.... (same roots as ancient German Keil = wedge) Schlüssel

key 2.... (geographic) e.g. Key-West, syn. = cay

cay ..... (geographic) (roots Celtic!) Sandbank = sands, sand-bank; Riff = reef; Inselchen = tiny island

Conclusion: The Celtic (Galicia in Spain) - Spanish - Caribbean root must be the right one.
