Go to the Five Island, and talk with the door, solve the puzzle, defeat the Rocket Grunts and get the Sapphire.

"Up, Left, Right, Down"
Fall down the holes in that order to reach the Sapphire.
The braille inside this room says:
"Let the two glittering stones,

This basically means that once both the Ruby and Sapphire are given to Celio, you can trade between Ruby, Sapphire, and Emerald.
