With degenerative disc disease, disc bulging occurs with flattening out of the disc and subsequent spreading out. Typical bulging presents symmetrical, but sometimes a protrusion will develop that is eccentric, ie., asymmetrical bulging. An eccentric protrusion will be to one side, in this case left, sometimes causing neurologic symptoms on that side, sometimes not. A protrusion is less than 3 mm, whereas a herniation exceeds 3 mm.

There are 5 Lumbar 'levels', labeled L1 to L5. These are lower back disks.

Each spinal level has a spongey material between the spinal vertebra (the spinal bones). Because it is 'between', they give 2 'levels', such as L4-L5. When the spongey material 'leaks' it can create a disk bulge, or if it 'breaks open' it can create a total herniation. A disk bulge can be (typically) posterior, meaning to the back. Or lateral, which means to the side, right or left. Asymmetrical means, loosely, a non-even shape.

So your question can be rephrased as a non-even disk material bulge to the left at Lumbar 4 - Lumbar 5.

NOTE: Many people have disk bulges without pain, though other people do have pain.

What is sciatica how it occurs?

Some sources of sciatica:Lumbar disc - this includes disc protrusion that compresses the L4 or L5 nerve root as it exits the spine. The outer fibrous layer of the disc weakens with wear and tear and eventually allows the inner jelly-like nucleus to bulge out and irritate the nerve root. The presence of a disc protrusion can be confirmed by specialised imaging such as CT scan or MRI.Lumbar joints - in some cases misalignment of the spine causes inflammation, irritating the nerve roots and producing sciatica. Your chiropractor will perform tests to determine if the sciatica pain is more disc or joint related.Muscular - the sciatic nerve passes through, or in front of, the piriformis muscle, deep in the buttock. Spasm of this muscle due to pelvic imbalance can compress the sciatic nerve - a condition known as Piriformis Syndrome.Only rarely is sciatica caused by serious pathology that requires medical referral. The majority of sciatic cases respond well to chiropractic care and specific exercises.
