Frederick Fleet was on lookout duty the night the Titanic sank. He saw a foggy figure out ahead of the vessel and rang three bells (the alarm). He called up the bridge (place where the ship is steered from) and said "ICEBERG, RIGHT AHEAD!" He was 24 at the time and survived the sinking by getting on Lifeboat #6.

I don't believe Frederick Fleet was blamed in the inquiries following the Titanic sinking. One of the factors however, was the lack of binoculars for lookouts like Fleet to use. It was believed having those binoculars might have helped them see an iceberg sooner, but experts later say that it would not have made much of a difference considering the conditions at the time: clear but moonless night, very calm waters. It was still difficult to see breaks in the water whether you have binoculars or not.

The lookout crew manned the crow's nest up high on the ship's forward mast. Their job was to look for any obstacles the ship might run into.
