If the heart is injured or diseased, the organism is likely to die without medical intervention because the heart pumps blood to all parts of the body and without blood none of the organs will work.
If minor injury then it will heal, during which period the animal will have a reduced capacity for work.

If major, it will die.

It depends on the severity of the damage, but it has the potential to cause death to the organism.
The problem with this question is that you haven't actually told us what "these parts" are. But leaving that aside, damage to the parts of the heart will generally cause the heart to stop beating, which results in the death of the organism (unless there is serious medical intervention, which might be able to keep the organism alive, depending upon the nature of the injuries).
Depending on the injury and the severity of the injury, a person might stay alive, but may have to limit activities do because of the condition. If the injury is serious, an organism will die, though sometimes something can be done medically for humans. For animals, the cost of doing anything medically is often too costly and the animal would usually be put down.

Depends on the extension and the place in the heart where the injury occurred.

But the chances of survival are minimal.

It depends on which parts - some people live long lives after a heart attack damages their heart tissues, while others die almost immediately.
