Some possibilities - although not exhaustive - are a lung infection, broken bones, and a subluxation in your back...a chiropractor can advise on and fix the latter.
Damage to the long thorasic nerve can cause this kind of pain as well. It affects the muscle that is responsible for keeping your shoulder blade in place.. This can cause the other muscles around it to go into spasm from compensating. The injury can happen by harming any part of the nerve which runs from the base of your neck/skull and deep into the back. The pain is not usually present where the injury occurs. To see if this is what is wrong, stand about an arms length from a wall and put both hands on the wall. Lean in to the wall like a push up, very slowly. Try to look in a mirror id possible, or have someone else feel both shoulder blades as you go into the wall and slowly push back out. If one blade seems to be winged out more than the other at any point in this, then it is very likely that this is your problem. See a doc and ask for a neurological check up. I suffer from this injury, it very often feels like a lung issue but is not actually related to the respiritory system.
