Don't know when he died but can tell you how he did. He died because he just used three techniques of insane power, and used up every ounce of life energy in his body.
He had an internal illness, that was killing him, and the overuse of the Mangekyou Sharingan drained the life out of him also along with making him blind,
That's wrong,he wasn't blind.And he died fighting against Sasuke(Naruto Shippuden).The illness thing did make him weak.But he died because he used a techniqie call Susano'o,and he took a risk by doing so.Eventualy he died because of that.

Itachi died because of an over usage of his Mangekyo Sharingan. And of an illness. Itachi chose to die instead of killing Sasuke.

Itachi still had eyes in his corspe when Madara took his body and Madara extracted Itachi's eyes which has now been transplanted to Sasuke recently. Sasuke now will have eternal mangekyo sharingan.
