Ahsoka was born on Shili, the homeworld to her people (the Togrutas). She was brought to the Jedi Temple by Plo Koon when she was 3 years old. As for what happens to her, Ahsoka's fate has not been confirmed yet. There are several possibilities of what could happen to her. We'll just have to wait and see.

In my opinion, it probably happened during Order 66 when anaking was killing the Jedi at the temple. Now, Ahsoka might have not been at the temple. She could've been at a clone war destination and killed by clones. But Ahsoka's fate has not been confirmed just yet. So we'll have to find out.
