The person who write a short concept of a game, then sends it in to a publisher, if it gets accepted the game designer will write a very detailed design document that details every single thing about the game; script, items, characters, location of every single Non-Player Character (NPC), item, building, etc. Very detailed, because a team of graphic designers and programmers is going to build you game from that document while you start writing another game design document.
