The ghettos did not necessarily fail, a lot of the time there was resistance or revolts took place, which threatened Nazi control and sometimes even lead to the liberation of tens to thousands of people. A good example of this would be the film Defiance, which is about the true story of two men, the Bielsky brothers, who lead people from their town to the woods and lived there for years while the Holocaust was going on. They saved thousands of people from the ghettos, deportation and placement in camps this way.

The Warsaw Ghetto uprising was actually a relative success. Jews without any serious armament were able to resist Wehrmacht (German military) forces in the confined space of the ghetto for longer than the entire Polish Army was able to resist the Wehrmacht. However, as to be expected, when the Germans moved entire tank divisions and armored personnel into the ghetto, the uprising was quelled. A few people with molotov cocktails could not really hope to defeat a massive army. Anielewicz and other leaders of the uprising committed suicide when it was clear that defeat was imminent to avoid being tortured. Their bodies were never recovered.
