There is no record of Cleopatra's agenda when she was visiting Italy so there is no way for us to know if she traveled on the Appain way. If she entered Italy at the port of Brundisium, yes, she would have used the Via Appia to get to Rome. However, if she entered Italy at a place like, say, Ostia, she would not have used the Appain way.

The Roman empire was a vast expanse of territory and they had ports wherever there was a waterway from the North Sea to Africa, from the Atlantic to the Black Sea. Some of the more famous ones were Regium, Alexandria, Ostia, Brundisium, and Tyre, but there were many, many more.
