Actually, a concerto can be for almost any solo instrument and
orchestra. The commonest are those for piano & orchestra,
violin & orchestra, and cello & orchestra.

The conductor, if there is one; otherwise, the soloist (in a
concerto) or the leader of the orchestra (=the leader of the first
violin section) will set the tempo.
