General information

Other names

Human leukocyte antigens, organ donor or transplant tissue typing, lymphocyte cross match, and histocompatibility testing.

Purpose
  • To identify surface groups of proteins known as human leukocyte antigens (HLAs), which are unique in each person.
  • To match HLA types of organ donor and recipient, which lowers the risk of organ rejection.
  • To help determine paternity, since HLA systems are inherited.
  • To help diagnose ankylosing spondylitis and sacroiliitis, which are often associated with an HLA called HLA-B27.
How it works

HLAs detectable in blood provide clues to immune conditions.

Preparation

None.

Test procedure

A blood sample is drawn and sent to a laboratory for analysis.

After the test

Follow procedure for drawing blood.

Factors affecting results

None.

Interpretation
  • If done for organ transplantation, HLA type will be compared to those of available organs.
  • Blood may be analyzed for particular HLAs that indicate disease.
  • To determine paternity, a child's HLA type is compared to that of a possible father.
Advantages
  • There's no risk.
  • It improves results of organ transplantation.
Disadvantages

None.

The next step
  • After a positive test, if done to confirm ankylosing spondylitis or sacroiliitis, X-rays of the spine or sacroiliac joints may be done and treatment begun.
  • If done for donor matching, transplant can proceed if match is good.
