Pressing the R button will make your character block.
If you do not block on a particular stage, when you finish it, you will come up with the achievement, "No R for you" (You did not press the R button once).

Little stars indicate how many KO's you have made in Melee mode. The stars change if you change your name to one of your created names. Try to get as many stars as you can.
In the Subspace Emissary in Super Smash Bros. Brawl, the crowns mean you have completely explored a level.
Do you mean Super Smash Bros 64? You can get one from Amazon, but you won't find it in a store.
They are already unlocked in Super Smash Bros. Melee. Unless you mean the trophy, you need to complete Classic, Adventure, or All-Star Modes to unlock their trophies.
