Open the glove box and take the insert out. It should be behind that on the right side.

I'm not a mechanic / technician but I believe that the 4x4 Control Trac system on a 1995 Ford Explorer does not have a separate control module I believe the 4x4 system is controlled by the GEM ( Generic Electronic Module ) and that it is located in the center stack of the dash near the radio
The 4x4 control module is located behind the kick panel in the passenger side floor board. The module simply unplugs and you plug the new one in.

Yes the do it's part of the ETCM ( Electronic Transmission Control Module) and it connects to the engine.
A 1994 does not have a control module on the transfer case. But it does have a 4x4 Actuator on the front differential.
Could be the 4x4 Control Module.

Yes - looking at the owner's manual , it shows a 5 amp fuse in location # 12 for the 4X4 module and foglamp switch
