My first and best guess is 'depression'. Especially the part about, 'always tired'. You eliminate 'always tired' by going and exercising daily to the point of sweating. If you don't do this (and I haven't once or twice) it is unbelievable how fast you lose muscle strength and energy/motivation. You feel sick inside as a result in many cases. The result of not exercising is very often depression. I wonder if the man is angry. Anger kept inside is often a trigger for depression. Being a smoker is not good, but may not be the issue. Watching TV all the time has been proven to deteriorate your brain, and obesity can squeeze the heart, making it work harder. (And making you tired).
