If you are referring to a pre-paid card, then no. Your Xbox will tell you that the points are not valid if you use foreign points.

The xbox's are exactly the same, but the elite is just a bundle package and you get everything you need to play online (worth your money if you plan on playing the games online)
It depends how you think of it: in total it is a lot of money, but the gaming community on xbox live is massive. I personally believe that you can get the same online experience through sony's PSN.
