First the term shemale is VERY derogeratory to almost everyone it applies to. It was created by the porn industry, and is used almost exclusively there.

Now, that that is out of the way. A "shemale" is defined as a person with a female body but male genitals. Prior to puberty only the genitals differentiate between male and female. As such no one can be born a "shemale".

There are basically 2 groups that can fit the description later in life though.

The most common being MtF preoperative transsexuals. The physical changes in this case are induced artificially, through hormone therapy.

The other being an intersexed persons who physically appears male externally, but goes through a female puberty developing breast. This is as close as you can get to being born a "shemale".

The term she-male is offensive to transgendered people. It is often used in the sex trade or porn. The person has male genitalia with breasts that have been enlarged using hormones and/or implants. (All males and females are born with breasts). It is best to understand that these are transsexual people who have transitioned to female but have not undergone genital surgery. It doesn't matter if they are born this way or not but how they feel their sexually is.

No. "Shemale" persons are born as normal, healthy males. But then they get into kink or fetishes and somehow get hired to do pornography in drag with their male genitals exposed, and they take hormones and/or surgeries to the breasts and face to make their bodies match their pornographic part more.
