If the grout is dirty there is a chance that the new grout will not stick over it. If you clean it out first the likelihood of it working increases, as well as looking better in the long run.

Do you seal tile before you grout?

It depends on the type of tile you have. If the tile is unglazed and porous (as mostly all travertine is) then you will need to seal the tile before you grout it so that the grout does not soak into the tile and cause it to become dull. Follow the manufacturers recommendation for the tile that you have.

Can water be leaking through the grout of a second floor bathroom tile floor?

== == Water can very well find it's way through the floor. It is usually at areas where the tile meets a vertical area at a shower or tub. Movement can occur in these areas. It is best to caulk joints such as these, instead of grout. The caulking will flex, but the grout will crack and allow water to penetrate.
