Did you break the clutch cable or the clutch disc?
Clutch cables break because they just wear out. The axle can't do that.
What normally causes clutch discs to fail is overstress: you pushed the clutch in without letting off the gas, downshifted at too high a speed, or souped up your engine without putting a stronger clutch in the car. Axles can't do that either.
After repairing the cause of the code, you clear with a scan tool.
A slipping clutch may cause cracks on the flywheel wear surface from excessive heat. The cracks would be reason enough to replace the flywheel when replacing the clutch assembly.

Break, as in come apart in several pieces or the outside ring came loose from the center. In the first case if there was no damage to it then a bad casting would probably be the cause. In the second case, over reving or dumping the clutch could be the cause.
A worn out clutch release bearing would cause a whining sound when you put your foot on the clutch.

Take off the caliper. Then the saddle. There will a screw holding the rotor on you will have to take that out. The best way to take it out is to buy a impact driver. But if you dont have one then just get the right screw driver and hit it with a hammer this will cause the screw to loosen.

There is no hydraulic fluid in the transmission. However, there is hydraulic fluid in the clutch system. If the clutch fluid reservoir is empty it would cause clutch not to operate.
