You could fine an employee if it was written into the employee handbook and the handbook was part of the employment agreement. In other words, the employee agreed to these conditions when taking the job. You can't just impose a fine because you felt like it.

What is the fine for not wearing a safety vest on a forklift?

That depends on what country you are in, and perhaps also on company policy. In the US employees are subject to company policy; OSHA cannot issue fines to individual employees - only to employers. However, in general there would not seem to be a safety advantage to wearing a "safety vest" when driving a forklift, so this may not be necessary protective equipment and no fine would be imposed by anyone.
