Being a witness at a trial is not like being a student in school. A doctor's note will not automatically excuse you. At the very least contact the attorney that subpoena' d you and notify them. You'd better be plenty sick or injured. If you're subpoena'd as a witness in a trial I would NOT fail to show up to answer the subpoena and later hope that the judge will forgive you.

Referring to downloadable legal forms available on the internet and guaranteed to be valid in PA; no witness signature is asked for nor is a space provided for one. Interestingly enough, neither does there appear to be a requirement, or space for a Notary Public's signature.
That document is a subpoena.
Usually, witnesses are issued a subpoena and they come of their own means. If a witness does not want to appear, law enforcement can be directed to bring the witness.
