Although some factions of some societies accept various declarations of sexuality (compare "gender" with "sex") the considerations are either cosmetic or superficial or in some cases just self-declared and change from day to day (or indeed, minute to minute). However, the phenotype of the human remains the same (either male or female - that is sex, not gender). Having said that, there are legitimate degrees of sex within a phenotype, such as XYY males and XXY females, and so forth. However, even in these cases, the phenotype remains the same even if the individual has elective surgery. Since physical sex is being studied and it is being found that phenotype and genotype are not in perfect alignment when it comes to human sexual organ systems, under some conditions even XX and XY are not perfect indicators of phenotype. Although at present time sexual chimera are a natural oddity (extremely low chance) with advances in medical science it will be possible to generate working organs and replace present organs. Sex is not just about the parts hanging between your legs, or the shape of your chromosomes; it also relates deeply to environment, self-perception and chemical make-up. Although it appears to be confusing, there is no proven harm in treating people as the sex they perceive themselves. Follow the criminal and psychological histories of those having gotten either GRS or just lived as the opposite sex (role.) You will find that in most cases, the patient/client lives as well or better after being 'accepted' as the opposite sex. If there was something actually wrong with accepting those whose self-perception doesn't match our perception, then you would likely see a lot of abnormal behaviors (in relation to both sexes) versus what actually is happening: i.e. acting out, looking to remove even more tissue, even "worse" announcements { An exaggeration here but - "I'm getting a species-change"}

Sex is deeper than both pheno and geno-type. And it is extremely rare to find any one person who is comparatively sane AND gender-queer to change their perceived and announced sex day-to-day much less minute-to-minute. Those individuals showing these characteristics are usually broken into highly disturbed persons with much worse problems than sex vs gender vs orientation and those under mental and possibly physical pressure form society and family to fit in and match what society deems as correct and cause them to vacillate because they can't thread the needle of individual vs group good. In 99.99999% of the people in the United States, they are one sex that does not vary even if that sex does not match what others proclaim is right or wrong.

Yes almost completely. By adding male hormones the body will change and removing the breasts. The final operation is making the male genitalia. That will never be exactly as a male though.
