Probably not. It really depends on your definition of severe. Some people feel that they have severe symptoms but in reality their NCS (nerve conduction studies) are reported out as mild and a large portion of their symptoms are related to tendonitis etc..However, If you actually have NCS/EMG studies showing severe CTS and symptoms for longer than 3 months, then you will need carpal tunnel release

Now this is very possible to treat carpal tunnel syndrome naturally even you need not to do surgery also. You can do physical exercise and take little bit of medicine. In terms of 95% people has the chance to be ok from this problem without surgery. that means naturally you will be okay.

It probably will depend on your definition of what pain is. Generally the most common symptom or symptoms are numbness and tingling of the first 3 digits (thumb, index, and middle fingers) causing your hands to feel like they have fallen asleep. Due to this, a lot of people think they have a circulation problem, but in reality it is a neurological problem.

Some people will also describe and ache or throbbing of the hands that might be considered "pain". You can also have hand/wrist tendonitis along with carpal tunnel syndrome which may give you "pain" in the hands etc..

Most people have a combination of numbness, tingling, and pain with carpal tunnel syndrome. It is possible to have carpal tunnel syndrome without pain. Some patients complain only of numb fingers.
