Because by definition a birthmark is a mark present at birth, no, you can not get a birthmark later in life: "moles" frequently, in fact typically, develop later in life, but are not birthmarks.

The "Becker's Nevus" (a brown patch, often hairy, and/ or freckled; typically located on the chest or upper arm or back) which usually appears during puberty/ early adulthood in a minority of males (some studies assert as many as 2% have them; other studies give a much lower frequency - under 0.5%) is, however, regarded as a "birthmark" because it seems genetically determined.

It is probable that the emergence of such marks on a male's skin is triggered by the circulation of large amounts of testosterone during puberty, but the exact mechanisms involved, and how and why a small percentage of males are so affected, are currently imperfectly understood. There does seem to be a "genetic marker" for such nevii in the DNA of all men bearing these marks who have been studied.

Yes, it is possible, because the birth mark can start to form, if it hadn't finished forming while the baby was in the mothers womb
