George W. Bush can not be elected vice president because the Constitution allows a maximum of 10 years as president, and the Vice President must be able to replace the President for the remainder of the term if they are unable to serve. If the vice president passed away with less than 2 years remaining in the term, it is plausible that George W. Bush could be nominated as Vice President, but highly unlikely and against unspoken convention.
No- you have it backwards-- Washington refused to serve a third term even though some supporters urged him to seek another term.
Franklin Roosevelt was the only president to serve or run for more than 2 terms. He died before he completed his fourth term. George Washington probably could have served for as many terms as he wished, but he refused to serve a third term and that set a precedent which everybody honored until FDR broke it. Now there is a constitutional amendment that disallows a third term.
