Your character will not look exactly like the box art, but you if you become evil and corrupt enough you will look pretty rough. I played through as good, but my husband is currently playing through as evil. His corruption and evil levels are both topped out. He has horns that are as wide as his head out of each side along with some other wicked looking features. Your alignment also has an effect on how your dog will look. Mine was more like a golden retriever, whereas his is more like a black lab.
