Can you die from a fail?

If one has failed to keep themselves onto a building or cliff, or had failed to hold their breath underwater, or failed to put a fire out, failed to remember a parachute sky diving, failed at running from an angry and large animal, failed by being near metal in a storm and got struck by lightening, most likely. Basically it depends on what they failed AT. If they fail to stay alive, they die.
