If its a viral infection, the body will fight it on its own, and its extremely unlikely that it can be complications.
If its a bacterial infection on the other hand, it can. But for this to happen it has to remain untreated and the infection has to spread to the jugular and can cause sepsis.

if your throat closes up to a certain extent then you wont be able to breathe, and if it really hurts you it could stop you from eating and drinking causing you to be starved and dehydrated, also if you dont treat it right quick enough it could spread around the body.

i do not have the complete answer for many people out there questioning this fact. I know that I have had tonsillitis for now 5 years and I have no gotten it removed. I do still get the pain and everything but there are times it just does not hurt any longer. So when I research it more I will give you a more better answer to this question we are all wondering about.
