Yes. There are a number of causes of myocardial infarction (heart attack). You can have a blood clot block arteries in your heart, you can have plaque dislodge and block arteries in your heart, or you can have a rupture of arteries in your heart. Basically, anything that blocks adequate blood flow to the heart's muscle will cause an infarction. Having normal blood pressure only reduces the risk of one possible way to have an infarction: rupture. It does not, however, remove the risk all together.

Yes! There are numerous studies, especially ones from the 1950s and 1960s that showed that patients who had died from a heart attack did not necessarily have blockages in their arteries. We also known of a couple of medical conditions that cause chest pain that are not due to blockages. For example, the coronary arteries, the arteries that actually supply the heart tissue with blood, can actually spasm and reduce blood flow thus causing a heart attack.
