You have to get a man and a women to relax on a double bed. Click on onw of the sim with the other on. Choose the option of whoo hoo and try for a baby. You may not get pregnant right away so you may have to try several times. When you hear a chime that means you girl sim is pregnant. Or I you whoohoo and that option is gone, and you can also tell when you sim is pregnant when you female sim runs to the bathroom and is sick. Also sims can have one child or twins. Sims wont have an option to whoohoo is the female is too old or if they don't have a good relationship with each other. For same sex partners or if you wish to adopt you can use the phone to call the adopt services.
