What happened was, when the cranks were put on, they were not torqued correctly. After a while the crank arm starts to rock on the spindle causing wear to the splines in the crank arm. Once this wear has taken place, the arm will be almost impossible to keep tight without something like JB Weld or locktite on both the splines of the spindle and the threads of the lock bolt.
