Experienced professional piercers who know there stuff always do nipple piercing with barbells, being a straight piercing a barbell will allow for normal movement without the risk of snagging. They are far more comfortable during the initial healing stages then once the piercing has a good light heal (6~8 weeks) you can change the jewellery to a "D" ring with will offer the effect of a ring with the comfort of a barbell. Research your piercer and make sure you are dealing with an experienced piercer and not a novice piercer, you won't be disappointed.
Sure as long as the piercing is healed and seasoned you can get it stretched up at any body piercing studio. I would not suggest trying to do it yourself so you don't tear the tissue. Ouch

Well a nipple barbell is measured from inside the first ball on the shaft to the other ball inside on the shaft. It's the inside measurement you need not including the thread on balls. In a nipple (male of female) the barbell should be 2~3 millimeters longer than the actual nipple is across to ensure there is room for cleaning and enhancement. The nipple will become larger in diameter as the piercing heals, this applies to both female and male nipple piercings.

Well if you still have the barbell and you can get it back in the piercing without damaging the piercing, then put it in and tape it down till you can get thread on balls for the ends. If you think you have issues of not being able to get the barbell back in go see your local body piercer and have them put it in for you. They can taper the piercing and put the barbell back in without damaging the piercing.
first of all, when you get a nipple piercing, make sure you are putting a straight barbell in, instead of the hoop. putting a hoop in right away usually results in Migration (meaning; the skin will reject the piercing and eventually you'll see fewer and fewer skin covering the jewelry til there just isn't any. ). You can keep your navel piercing until you start to show, then see your local piercer and get a flexable barbell put into the piercing. It's call a PTFE barbell and it will allow for the piercing to stretch without tearing the piercing. However be sure you are getting a PTFE barbell and not some other material. PTFE is biocompatable and will be accepted by the body without issues, I have used these for hundreds of my clients without problems.
