yes it does matter what address you put on a job application, the address you put on a job application is supposed to be where you reside, no where else.

Most people have had jobs of some kind, even if they are not paid by the hour. The work they do for the neighbors, working with your mom or dad around the house doing chores etc. If you really have never helped out doing anything then you can fill in the section with " This is my first entry into the job market, personal references can be supplied"

I wouldn't put the reason on the job application. You can save this for the interview and only if asked for the reason why.

You need to put on the application that you left job to be at home with your children. No employer would down upon this.
