yes it does matter what address you put on a job application, the address you put on a job application is supposed to be where you reside, no where else.
Application for what? Commercial Driver's License - job application - security clearance - what? How is the application worded? Does it ask you to list ALL arrests and/orconvictions? If so, you'd probably better. If you don't, and they find out later - all the worse for you.

A DUI conviction on your DMV driving record NEVER goes away. Your driving record is a compilatioon of your entire driving history. To be safe you should include it. If it is 20 years old and you haven't had any subsequent offenses it probably won't count against you after all this time, AND you may possibly get points for honesty from a future employer.
