Light or moderate cases of knocking will usually resolve themselves when you switch to a fuel of the correct octane rating. Severe cases of knocking can knock pieces out of the pistons, which of course won't go away w/o an engine rebuild. Running unleaded fuel (of the correct octane rating) in an engine not built for it will usually eventually damage the valve seats, and will require a cylinder head rebuild to fix.

Lead used to be added to gasoline to stop pre-ignition or detonation (which causes engine knock or 'pinking') but it was found to have very bad side effects environmentally and physiologically. most countries have banned the use of leaded gasolines, and modern gasoline have other ways of dealing with detonation. so i guess, unless you like bad air and bad health, unleaded is best.
