First of all Resident Evil 4 is not actually the fourth game in the series, Resident Evil: Code Veronica is, and secondly Resident Evil: Afterlife is actually mainly based on elements from Resident Evil 5(technically 6th game in the series). So the major difference between the two is the location, and characters. Resident Evil 5 occured in Africa, and pairs Chris and Sheva against Wesker, in the movie Sheva is replaced by Chris's sister Claire, but all involved still face Wesker, and the film mostly takes place in LA. The las plagas creatures are in the game but are explained to be further zombie mutation, and not a parasite like in the game, the Axeman makes an appearance as well as the las plagas dogs like in the game. Jill also appears in both as being controlled by Wesker with the spider like device, but is not as involved in the movie only appearing in the final scene. Another difference is Wesker dies in the game, but not in the movie, you see him survive if you pay close attention to the scene where Luther comes from the drain.
