You will get signs such as abnormal shape and enlargement, mine went to the size of my fist your doctor would then take bloods tests to check your tumour markers. If they suspect cancer a biopsy will be then taken and removal of the testicle

Once a month, a man should do a testicular self exam to check for any anomalies. It is best to do this at the end of a long hot shower, standing up. Simply take your fingers and gently examine the surface of each testicle. They should feel like a boiled egg without the shell. At the top and back of each testicle, it should feel like there are connective tissues which some people say feels like cooked spaghetti. If you feel any lumps anywhere else, you should see your doctor. There is typically no pain or discomfort with testicular cancer in the early stages, so a self exam is important.

As a rule, testicle pain is not usually an indication of testicular cancer because the presence of a tumor is not usually painful. You should regularly do a self examination though to make sure you do not have any lumps.

If left untreated, it WILL kill you. Probably within a year or less. But the good news is if it's caught early, it has a 98-99% cure rate and usually doesn't interfere with fertility.
