You must have an installation disc to upgrade Windows 98.
They are different things, if you have not yet purchased an upgrade disk you should invest in an install disk. Otherwise you will have to upgrade from Vista to windows 7 every time you want to reinstall.
you cannot upgrade from win95 directly to xp without a fresh install, if you have purchased an upgrade disc, you need windows 98 or newer. If you have purchased a regular install disc, back up your important files, and boot from the install disc, you do this by setting the BIOS (in "setup", before the O.S. starts to load)

No you have to buy it installed into the computer unlike windows which you can upgrade with a disc (I did when windows 8 came out).

It Depends What You Want To Upgrade, If you wanted to upgrade from say windows vista to windows 7 then YES you would need a disk, however if you were downloading the software to upgrade then you could put it onto a USB flash drive and install that way, some OS Like LINUX provide a installer which you can download from their website

You first need to check your computer can run windows 7. Then one way you can do it is through windows anytime upgrade which was introduced in Windows Vista. type upgrade into the start menu and press enter. Or you can buy the disc box set from any good computer store or supermarket and follow the instructions provided to install.
I think you can only use a windowsbootable floppy disc for the system in which it was made from.
