Presumably in this context it is your employer providing you with health insurance.

It is not that your company would be penalized, it is that the insurance company would refuse to pay for treatment of the preexisting condition. They set rates according to the known health of their pool of insured people. But upon signing up for your company's insurance plan you failed to divulge a known condition, and signed the statement saying you were giving the insurance company an accurate statement of your health, so you lied to them (aka committed fraud). That generally voids your policy.

Check the particular wording of your own insurance policy regarding preexisting conditions.

Is ADHD a pre-existing condition when it comes to Health Insurance?

If one was applying for health insurance and had to list existing conditions and ADHD had been diagnosed prior to the application it would be pre-existing. If it was diagnosed after the application and after the insurance is in place it would not be considered pre-existing, even if it did exist, so long as it had not been diagnosed by a doctor. Subject to change by someone that really knows!

If you were fine when you applied for coverage then had some problems but no diagnoses can your insurance say you had a pre-existing condition?

They can try and say that, but as far as I know the burden of proof is on them. If you told the truth on the application - they will have to pay. You might have a condition that the insurance company thinks a reasonalble person would have known that they had.

Could you get free health insurance with a pre existing condition?

Having a pre-existing condition can be tricky, when looking for health insurance. If you are unable to work, I would first contact the Social Security office and see if you qualify for Medicaid insurance or Medicare insurance. Each company is going to be different, so you will need to ask each company to see if the pre-existing condition will be a problem.
