In 1975 there was a TV show called "Switch!" this was a disaster for TV stations because the standard command for changing video sources in a control room is the word, "SWITCH". If a control operator (Usually a TD - Technical Director) was not paying attention, when that show came up on air, the TD might have a reflexive action and hit the TAKE button on the console! That could be very embarrassing (and possibly, costly!). With today's automation, that is not likely to happen!
