As long as you are having periods (even sporadic periods) you can get pregnant. Even though women are into menopause they can go several months to a year before having another period and still get pregnant.

If you are still ovulating and having your monthly period, it is possible. However you need to weigh the possibility that after you reach about 35, it may not be safe for you or the baby. Women are born with all the eggs she'll ever have and the older we get, the older the eggs get. Several birth defects can occur when an older egg is fertilized. In your 40's, while it is still possible to ovulate and become pregnant on your own, less than 10% of women in this age group who are trying to conceive may get pregnant in a years time, but over 50% of these pregnancies will end in miscarriage. So in fact at 45 the chance of becoming pregnant and carrying a pregnancy to term without medical help, is almost nil.

As women age the quality of eggs goes down rapidly, for this reason most doctors advise women over forty who wish to conceive, to use a donated egg. The donor is matched by physical characteristics, such as height, build, hair and eye color, education, talents and interests. Some clinics will even match donors by religion.

If you are in menopause, you will be given hormones so you start having cycles again. Once a egg donor match is made, the doctor will coordinate your cycle and the donors cycle. When the donor is ready to ovulate the doctor will remove the ripe eggs, they will be fertilized in-vitro with the sperm of the intended father or by donor sperm. After a couple days in the lab, where the egg and sperm are checked for good growth and viability, the doctor will place the fertilized ovum (called a blastocyst) in your uterus using a thin pipette. If all goes well the blastocyst will snuggle into the lining of your uterus and in a couple days implantation will occur.

Because women in their forties are at a slightly higher risk of miscarriage, or failure to become pregnant even when donor eggs are used, the doctor may use hormones to hyper-stimulate the donors ovaries, so multiple eggs will mature at once. They can be harvested, fertilized and stored for future use, if you fail to get pregnant on the first try. There are some risks to the woman associated with this procedure, so it is a decision best left to the doctor and donor.

Another option now available is to "adopt" frozen embryo's that are currently available, left over by their biological parents, when they were done having children. There are currently thousands of frozen embryo's available. The process while still new in public forums has been used for years, it has some draw backs to using embryo's that have been frozen for some length of time; so ask your fertility specialist what route he recommends.
