Hamlet stabs Claudius with a poisoned sword, but only the tip is poisonous and it is dubious whether Laertes' poisoned tip cuts the king as King Claudius cries out for help claiming he is only hurt. Hamlet then holds him down and forces him to drink the last of the poisoned wine that had been intended for Hamlet, but his mother, the queen, drank it and died. Laertes says the king "is justly served; It is a poison temper'd by himself. [the king]" Implying Laertes' poison didn't kill Claudius, Claudius's poison killed himself, just as Laertes was slain by his own posion from the sword. So, Hamlet kills Claudius by making him drink a cup of poisoned wine that Claudius had intended for Hamlet.

Hamlet's mother died by drinking a goblet of wine that was supposed to go to Hamlet. The King had poisoned the wine in an attempt to kill Hamlet and seeing his queen about to take a sip, he tries to persuade her otherwise. She drinks anyway.
First, Laertes stabs Hamlet with it. Then Hamlet takes it from Laertes and stabs Laertes with it. Then Hamlet stabs Claudius with it before forcing the poisoned wine down Claudius' throat.
Hamlet learns from the Ghost of his father that his father was killed by his uncle, who has since married Hamlet's mother. Hamlet decides to act crazy while figuring out what to do. His uncle Claudius sets spies on him because of his crazy behaviour. Hamlet confirms that the Ghost was telling the truth by putting on a play mirroring the murder of his father. Unfortunately this also tips Claudius off (he was pretty suspicious anyway) Hamlet catches and kills a spy in his mother's bedroom. Unfortunately it is the king's advisor Polonius. Claudius sends Hamlet to England. Polonius's son Laertes comes from France to revenge his father. Claudius tells him that Hamlet had killed Polonius. Hamlet doesn't get to England because of pirates, and returns to Denmark. Claudius and Laertes set up a Fencing match where Hamlet is to be murdered by an unbuttoned poisoned sword and a poisoned cup of wine. Hamlet gets cut by the poisoned sword but so does Laertes, and Hamlet's mother drinks the poisoned wine. Before he dies, Hamlet stabs and poisons Claudius to death.
Gertrude accidentally drinks from the poisoned cup, which Polonius had intended for Hamlet to drink from during his duel with Laertes. (In some productions, Gertrude knows that the cup is poisoned and drinks it anyway, to atone for her involvement in her husband's murder, or to save her son's life, or both.)
He has two plans to kill Hamlet. First, he sends Hamlet to England with sealed orders to the King of England to execute Hamlet. That plan fails. Second, Claudius arranges a fencing match of Hamlet against Laertes, where Laertes has a poisoned foil, and Claudius has also poisoned Hamlet's wine. That plan succeeds in killing Hamlet, but Queen Gertrude, Laertes, and Claudius also die, with Claudius killed by his own poison.
Laertes challenges Hamlet to a Fencing contest. Laertes treats his blade with poison and Claudius has a backup plan of poisoning Hamlet's wine.
Technically, King Cladius Kills Gertrude in Hamlet, but not intentionally. King Cladius poisons the wine for Hamlet to drink but he refuses it to keep fencing Laertes but when hamlet makes the second strike the Queen wants to drink to him and when she does she drinks from the poisoned cup and dies.
Hamlet is supposed to wait for Claudius "in the hall". The text does not tell us more. Where Claudius was in the hall (sitting in a chair, standing on a table, cowering under a sideboard) is up to the director. It is also up to the director to decide where on Claudius's body he got stabbed, although it cannot be anywhere that would be instantly fatal, since Hamlet afterwards feeds him poisoned wine. The arm, the nose, the crotch or anywhere would do.
Hamlet prevents Horatio from drinking what's left of the wine that Claudius poisoned, because Hamlet wants Horatio to tell people his side of the story.
