Umm... They ran away from the clans but the did not call the "clan" Heartclan At one point, they ran off together and formed a new clan. However, I don't think they called it HeartClan. They eventually realized their clanmates needed them and returned to their home clans. Hope this helps!

Um, no. They did in a fanfiction I read once...You must be thinking of that. However, they didn't make a Clan actually in the books.

Because he saved her in Starlight. In Twilight they ran off together, but Leafpool soon regrets the decision because she learns from Midnight that her Clan would be under attack. Leafpool and Crowfeather have an awkward and sad goodbye, and that's that.
