Uhm, there were NO heart surgeons during Colonial times! Physicians were one size fits all, with very basic education about anatomy and physiology. Medicine was herbs and plants. Surgeries were the very basic kinds--with high mortality rates. Antibiotics were not discovered until World War II, so Americans did not receive them until after WWII. As to salaries in colonial America... town physicians were often paid "in trade": a service for a hen, chicken, cow, eggs, hay, or slaughtered meat.
