1. Time. The multiple review processes can be implemented in a streamlined fashion or, if improperly managed, can bottleneck completion. There are a number of factors involved here, but projects are most affected when the evaluation period(s) are used as an excuse to add time or cost to a project in order to "filibuster" certain requirements, or when certain evaluation techniques become outdated. This is most often seen when using tools that, although properly calibrated, simply are outdated -- sometimes simply due to the fact that the new product outstrips these tools. Since the evaluation periods are structured into ISO 9001:2008 in such a way that everyone begins to view them as mandatory, it can be difficult to find work-arounds which will allow for deliverables to be posted in time.

2. There is heavy focus on customer feedback, but often not enough feedback from the workers themselves. This seems to sometimes be a large company, environment-specific problem. When workers are highly knowledgeable engineers and developers, with experience and skill (as they should be), and they alert management or D&D of potential problems, they should be listened to attentively. Unfortunately, in larger organizations, scale may prevent the message from reaching the appropriate ears. An easy solution? Some form of anonymous tip box. As old-fashioned as it might seem, a tip box implemented in the ISO style (with follow-up, documentation, and posted results) can be highly effective and a boost in company worker morale.

Extremely exhaustive process which takes a lot of time and resources.

Certification once completed does not gurantee 100% compliance as the organization takes it easy for a while after it leading to variation in performance.

I think ISO 9000 focues majorily on documentry evidences and superficialy touches on to product safety, purity and efficacy.
